How the Program Works
When you buy or lease any new 2010, 2011 or 2012 Chrysler, Jeep, Dodge, Ram or Fiat vehicle from a participating dealer, Chrysler Group will give you a cash reimbursement to help reduce the cost of installing the adaptive driver or passenger equipment on your vehicle. Leased vehicles must be leased for a minimum of 12 months to be eligible.
Once you have a 2010-2012 Chrysler, Jeep, Dodge or Ram vehicle that fits your transportation needs, contact a reputable and qualified adaptive equipment installer to ensure that it can be adapted to meet your needs.
Reimbursement
Conversions to Chrysler, Jeep, Dodge, Ram and Fiat models qualify for a maximum reimbursement of $1,000. Running boards qualify for a maximum reimbursement of $400. Alerting devices qualify for a maximum reimbursement of $200. These reimbursements will not be reduced or affected by any additional outside funding. Consult your dealer for complete eligibility requirements.
Steps for Reimbursement
1. When applicable, obtain a prescription or note from a licensed medical doctor on physician’s letterhead for reimbursement of the adaptive equipment. Note that not all applications require this action. See Application page.
2. Complete an Automobility Application. Forms are available at any dealership, adaptive equipment installer or download a printable PDF here.
3. After your vehicle has been adapted, submit your application and required documentation for reimbursement.
